PLEASE READ AND SIGN BELOW:

I, the undersigned parent or legal guardian, acknowledge and agree to the following terms on behalf of myself and my child:

1. Assumption of Risk: I understand that participation in activities at the indoor playground involves inherent risks, including but not limited to falls, equipment failure, collisions, or other incidents. I voluntarily assume all risks associated with my child’s participation.

2. Release of Liability: I release and hold harmless Skedaddle Kids, its owners, employees, agents, and affiliates from any and all claims, injuries, damages, losses, illness, death or liabilities arising out of or related to my child’s use of the playground.

3. Supervision: I acknowledge that I am responsible for supervising my child at all times. I will ensure that my child follows the rules and regulations set by the playground facility.

4. Photography Release: I consent to Skedaddle Kids using any photographs or videos taken of my child during their visit for promotional purposes. By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver and release of liability.
